The application process begins with the completion and submission of:
- the online application and
- two, signed, paper copies of the ORE 101 or 101A (only for course and administration projects)
with attachments to the ORE in NH 1024. The attachments may include: recruitment material, information letter, consent form, feedback letter, and a copy of the proposal if available, and must be submitted with the paper copies of the application.
The Forms for the ORE 101 and 101A are available via the ORE website.
First time applicants are encouraged to review the Guide to Completing an Application for Ethics Review of Human Research.
A Form 101A is completed for undergraduate courses and by UW Administration.
Both Forms 101 and 101A are accessible once you have entered your unique login and password for the online application.
Samples of recruitment materials, information letters, consent forms, and feedback/letters of appreciation are available at: www.research.uwaterloo.ca/application/101samples.htm. Researchers are encouraged to review and adapt these for use with their study.
Researchers conducting projects that involve participants (i.e., students and/or teachers) from the elementary or secondary schools in the Waterloo Region District School Board and Waterloo Catholic District School Board must also complete the corresponding version of Form 102 and follow the guidelines, as found in the School Research section.
