Appendix A: to be completed and included with each paper copy of the ORE for 101 if the research study includes the administration/use of a drug, medical device, or natural health product. Appendix A is a pdf file that should be printed and then completed. A copy of Appendix A is to be completed for each product included in the study.
Appendix B: to be completed and included with each paper copy of the ORE form 101 if the research study has an industry sponsor and/or involves procedures that pose greater than minimal risk to participants.
